
What the trials show
Women of about 75. In the TEST-III trial, 76 women trained for 18 minutes, three times in two weeks, for 54 weeks. Appendicular muscle mass rose by 0.4% while it fell by 1.5% in the control group. Leg strength rose by 9.8% against 0.2%.[1] Effects on bone density were borderline at the spine, and there was no effect at the hip.[2]
Men over 70 with sarcopenic obesity. In the FranSO study, 100 men trained for 16 weeks. EMS combined with whey protein improved the sarcopenia score significantly and reduced body fat by 2.1%.[3]
Pooled evidence. A meta-analysis of 13 randomised trials in people over 60 found moderate to large effects on muscle mass, strength and sarcopenia scores. It found no effect on waist circumference or blood lipids.[4]
Frail adults over 80. A small pilot trial found EMS feasible, with 88% adherence, under close monitoring of creatine kinase.[5]
Why EMS suits this group
Conventional strength training works well at any age, but many older people cannot or will not lift heavy loads, because of joint problems, fear of injury or lack of access. EMS produces strong muscle contractions with light movements and little load on the joints, in about 20 minutes a week.
The limits
- Every trial was supervised. We found no trial of older adults using a home suit alone.
- Participants were screened, and people with contraindications were excluded.
- EMS maintains and builds muscle. It does not replace walking, balance training or an adequate protein intake.
Practical advice
- Get a doctor’s clearance and show them the contraindication list.
- Start supervised, in a studio or with a trainer at home, and follow the build-up schedule of one session a week.

Is EMS training safe for seniors?
Trials in adults over 70, and a pilot in frail adults over 80, found supervised EMS feasible and safe for screened participants. Medical clearance is essential because several common conditions are contraindications.
Can EMS prevent muscle loss with age?
In a year-long trial of women aged about 75, EMS increased muscle mass slightly while it fell in the control group, and leg strength rose by nearly 10%.
